Why Am I Failing Emissions???
you just answered your own question.
the ECU set your code because it was detecting something wrong with the o2 sensor. so because it never came back on after that it doesn't mean the sensor fixed itself. there is something wrong with it. it's probably contaminated.
if you took your multimeter and measured the output signal of the o2 sensor I'm sure it will give you some weird readings........
change the sensor and retest the car......

$150 is about right for a generic sensor. Did you check to see if it'll work with your car?
And you are correct, you only have one oxygen sensor which is mounted at the collector of the header.

I found a Standard Motor Products Oxygen sensor, Upstream sensor, Vtec Engine for 170 or so, is that a specific fit for my car? Im a 95 tegra GSR. ACDelco Oxygen Sensor for B18C1 costs 98, and then their are some Universal Oxygen Sensor, Sensor Kits, B18C1 for like 65. But what is a sensor kit? and how is it universal if it is for B18c1? Can anyone shed some light on this for me?
I believe a "universal" kit requires you to cut some wiring off your old sensor and splice it onto the new one.
If you want something that will be a direct fit your best bet is to get something from a dealer. One such place is acuraautomotiveparts.org where it's I believe $180 or so. They're a dealer that sells parts online.
Otherwise NAPA has a bunch of options, the most affordable being a Beck Arnley direct fit sensor for $129.
