That's what Mal wanted. Remember Cobb promised that they would grow old together? It happened, and when they both died on the train tracks Mal woke up all crazy because of the idea that Cobb planted in her.
I didn't say she pushed Cobb off. She pushed Fischer off to get him out of Limbo, then jumped off herself. Cobb stayed in Limbo to look for Saito, but my guess is that because only Cobb knew he was in Limbo, he didn't age. However he did travel long enough that Saito was already old when he got there.
Tell us why you think he's still in a big dream. Oviously the movie was left open-ended for us to make our own presumptions. One big clue for me is the fact that his kids didn't age at all.

the biggest 'clue' for me was just a hunch, then little things like kids not aging, dad being there to pick him up, how saito and cobb got out of limbo (because weren't they still sedated and thus the gun to shoot them wouldn't do anything to get them out of those 4/5 levels?), and the way it ended showing the totem.. i know it wobbled then cut short trying to show it open ended, but irl if a top spins and wobbles it should fall over.
yea.. not that good but don't really remember too much either. i actually think i might watch it again in theaters, i think it was one of the very few movies i felt worthy of the ticket price

From a filmmakers' perspective though, I wouldn't have wanted it to be so blatantly obvious as Shutter Island, where Leo played a similar character trapped in a perpetual false reality.

i saw it twice and still had questions. but as for the kids, no one knows how long he's been gone from the kids. I mean i'm not expert on growth but if it was like say 6 months maybe they wouldn't change that much right?
Also the whole other thing me and my friends were discussing was about the the totem. How you are never supposed to have someone else's but Cobb always used Mals.
The whole thing with the kick's to get you out was strange at the end I agree, but as time slowed down it made sense for it to be easier as you went back up a level.

2. The idea of not using a totem is that so no one knows how its weighted. If someone (that was alive) knew how it worked, then they could architect a dream with a working totem. But because Mal died, I guess he could use it?
