I should point out that German cars are smarter than this. They hinge they pedal from the floor so that it's much harder to get something stuck behind it.

i remember this from 2 years ago on the ES350s. Customers would put the all-weather mats on top of the carpet mats not allowing the mats to properly be secured on the anchors. There's a note in big letters that clearly tells the customer not to install on top of existing floor mats(molded in the mat). They don't listen and this in turn causes dumb people to get the gas pedal caught. Natural selection if you ask me h:

yeah but in this case, it was a loaner vehicle.. and such "stacking" shouldnt have taken place.. if it was present

No! According to witnesses he was travling a high rate of speed from the highway 8 merge to the 125 "thats a good 5 miles from his death"
I would hope in all that panic he tried neutral:dunno:

Reverse lock would prevent that. But if you happen to break it you would probably end up in 1955 where theres a lack of *plutonium to produce the needed 1.21 gigawatts to bring you back. You would need something like a bolt of lightning.
