Second page of references, friends and coworkers are fucking worthless and nobody is going to call them. Direct Supervisors are best, people above them who actually knew who the hell you were are fine too.
Presumably this will be for submitting online, but if you get a face-to-face interview, go to Office Depot or whatever and print it on Resume Paper, put it in a resume folder, and put the folder inside the resume envelope. Tiny bit of money spent shows you're serious about presenting yourself.
