Don't hop on a bench press machine and go for it all. Get on the machine, add some weight, and focus on your form. If you're all over the bench grunting and squirming, you'll really hurt yourself.

well, when you're at the gym, take it light and find the machines you like. Don't do any workouts that require standing (arm curls etc), isolate muscle groups, and you should be okay.
