It is hard to get it to boost in 1st gear unless you floor it but every other gear, you shouldn't have a problem. It is normal for the manifold to smoke since it is burning off the outer coating. What color is the smoke blowing out of the exhaust?

The noise is the shockwave that is produced when the throttle closes. The shockwave is rattling the turbo wheel against it's thrust bearings. That is why you need a blow off valve. If you don't get a blow off valve, expect to rebuild the turbo very soon.

My idle is around 750 rpms. It fluctuates a bit because I have an open atmosphere bov that leaks at idle.

I don't know. The vacuum sounds normal, so it can't be a leak. How many miles do you have on the turbo now? Could have something to do with the IACV or something...

And it isn't -20 psi. Vacuum is measured in mm/hg, not psi.
