I've been trying to find those cases too but can't find. After some searching, those cases of rota failures were back in 2003 where they were having cavities in the ends of the spokes, but rota changed their designs and now VIA and ISO certified.

and ISO certification doesn't mean shit. ISO only outlines what minimum that is being done and guess who sets that minimum? that's right the very same company that has the ISO certification. it's all about having written procedures, doing what is written and being able to prove that you do it. It really has nothing to do with "quality"
