I've been trying to find those cases too but can't find. After some searching, those cases of rota failures were back in 2003 where they were having cavities in the ends of the spokes, but rota changed their designs and now VIA and ISO certified.

Because at least 18" is required for the 14" BBK that I'm going with

As far as the wheels go, most of the cases I saw online were 2-3 years ago. From what I remember, most of them also involved some sort of trauma to the rim itself (pothole, curb, etc.). I would imagine that a lot of higher end wheels would, at track speed also break if they hit a pothole or curb.
