could a doctor...
Have you taken your complaint to a physician or podiatrist? There may be something that can be done that's not as severe.
The #1 directive for all physicians is to do no harm, so I doubt that you'll find a bona fide surgeon who will just do it on your say-so. If you're diagnosed with a legitimate condition where amputation is the indicated treatment, then you might get your wish. But I think they'll want to try other therapies first.

what condition would require removal of said toe, and how do I go about acquiring it?
You could kick you bed post really hard...
But think about it: how often do you drop something on your foot that hits only your pinkie toe. Like if you drop a book on your foot, it'd be pretty hard to miss all of your foot except for the pinkie toe.
Stop doing drugs fool.

well, I don't see how a procedure other than removal could stop the toe from interfering in everyday life. like things falling on it. or getting slammed into by things. no toe, no pain.

i think your pinkie toe has more importance then you realize. it helps you keep your balance, you'd be off for awhile if you off-ed, don't get me wrong it's possible, just be ready to wobble around for awhile you sick fuck :fawk:
