Have you taken your complaint to a physician or podiatrist? There may be something that can be done that's not as severe.
The #1 directive for all physicians is to do no harm, so I doubt that you'll find a bona fide surgeon who will just do it on your say-so. If you're diagnosed with a legitimate condition where amputation is the indicated treatment, then you might get your wish. But I think they'll want to try other therapies first.
