Most likely i'd tell them. If would hate to wonder how things would have been if I hadn't said something. Might have been able to spend more time with loved ones, been able to do more things that i've always wanted, etc......

If I told them, it'll be an initial shock but won't come as a surprise when I pass away. Would hate to be here one day and gone the next and devastate them like that.
