Cell phone quest!
If you're going to go with that old, I'd look at rockin the moto v60 series. The old v60's were bricks of phones with metal cases, long battery life, good reception, and almost no extra features (not even games, which is a plus IMO). You can find them dirt cheap on eBay now, but you want to get a v60i, which has the stronger and better attenna design...
I wouldn't reccomend anything without bells and whistles. As long as you stay out of the menu it shouldn't hurt you. Just sign up for a plan and get the basic phone.

i am a dealer for verizon, sprint and cingular.
you can certainly get a non bell/whistle phone and a reasonably cheap monthly plan.
verizon has the samsung a650 and the LG vx4650 that are both non-camera and very basic and reliable. the 4650 has a speaker phone which is actually very useful.
sprint has the samsung vi660. same exact phone as the a650 for verizon, just carried by sprint.
cingular has the samsung x427m which i think is being discontinued, but again it is the same phone as the vi660 and the a650. small clamshell phone with no frills.
these phones are often free or $20 with a 2 year agreement.
as for plans, you're looking at about 40 bucks a month for 450-500 weekday during the day minutes. all plans include nationwide long distance and no roaming charges.
verizon gives you 450 weekday during the day mins, unlimited nights and weekends and unlimited verizon to verizon calling.
cingular gives you 450 as well, but they have rollover minutes.. this means that whatever minutes you do not use during the month carry over to the next for a full year. this plan includes unlimited cing/att to cing/att calling, and 5000 night and weekend minutes.
sprint will be 40 bucks for 300 weekday during the day mins, and their unique feature is what they call 'fair and flexible' which means that if you go over your 300 mins, they charge you 5 bucks for every extra 100 you need. this plan includes unlimited nights and weekends, and sprint/nextel to sprint/nextel calling is an extra $5/month

Check out letstalk.com. They have deals all the time where you get a free phone, or sometimes even net 50 bucks in your pocket after rebates.
If you don't want to use the gizmos then don't. I have a camera phone and while I do every once in a very long while snap a picture or two, for the most part all I do is make calls.
As for one-way billing, I highly doubt anyone does that anymore. Even the lowest-priced contracts have so many minutes that it doesn't really matter anymore.

The V260, V188, V180 and V60s are all basic phones with no cameras and are totally free. Some have rebates that you end up with money in your pocket. The V60s is the most basic of the lot, it doesn't even have a color screen, but it's nice and small and sleek. You'd probably like that one the most.

Nokia 8290
Small and lightweight. No color screen, so you can still see it under the sun (unlike color screens on phones today). Nice loud ringtones, plus you can't mistake it for a radio playing. No camera, and great battery life.
As for the kind of plan that you're looking for... Slim to none. And slim left 20 min. ago.
