Accident at work, what to do?
you should tell someone. take a look at what your friend is going through. you already missed a chance to stop something horrible from happening, dont miss the 2nd chance.

Just from what you've said it sounds like they care more about getting the job done, than the safety of their employees. If that's the case you need to say something. They are trying to blackmail you, and they should be kissing your ass. When applying for another job I would list coworkers (that are friends) as refrences, and not your old manager. I encourage you to speak out.
