Unless the B18 A/B engine is different from the B18C1, this is probably the root of your problems...
The FITV is located on the bottom of your throttle body, and the IACV is located on the rear of the intake manifold. At some point, the FITV and IACV were combined into one unit which mounts to the rear of the Intake Manifold.
If your old setup did not have the issues your experiencing now, try eliminating the FITV. If you still have the same issues, you might need to switch back to your old TB until you have a tunning solution.
