Probably warped rotors and/or they need to be turned bad. If they are that badly grooved and you find they are not warped, then you probably just need new rotors. Once they get severely grooved, turning them willl not help.

Look to see if your pads have delaminated from their backs as well. This can cause more than usually oscillation. However, I'd probably say it's your rotors at this point.
