Not necessarily, it depends on where the leak is in the gasket.

I had a head gasket go out on a 95 Neon, it would boil the water out of the overflow tank, intermittently. I had no oil in the water or water in the oil but it did put compressed air into the coolant.

It seemed to do OK around town but on long high speed trips it would overheat.
