Dear Mr. McPherson,
Jane Smith returned to work today after being absent for two consecutive days. She claims to have been ill and saw a doctor on April 11, 2003. She failed to produce a note from her doctor stating she was not fit to work. Ms. Smith was also absent on April 7, 2003 claiming to be too ill to work and also failing to produce a doctor’s note. Ms. Smith has missed more days than her allotted sick leave days. In total, Ms. Smith has been absent 8 times and has been tardy 14 times. I have enclosed a summary of the days she has been absent or tardy this year:

Ms. Smith lives 15 minutes away but she has had a poor attendance record to date. On March 24 she was put on medical certification. Since then, Ms. Smith has been absent 3 days and has failed to produce medical certification. I recommend Ms. Smith be terminated immediately.
