At work I use either "Sir," or "Ma'am," ... sometimes use "<rank & last name>,"

This kind of depends on the content of the e-mail. If I was sending an email because someone asked me for some information then I'd start it with this. But as far as formalities go...

It's about a meeting i attended yesterday and how during the question period i asked a question but I think i might have asked it incorectly. I did not explain myself correctly and i now want to bring forward an idea. It will porbably not be taken very seriously but i think that at least they should realize that we have some good ideas about how to acheive our comon goals which is profitability and garanteeing our survival during these very competitive times.

I'm definitively not the best person to send in the email but i figured i'd let them know my ideas anywai.
