i need to make a form for an apartment complex, that residents will use to solicit themselves to others regarding special skills (i.e. handyman work needed/wanted, computer help etc..) so other residents can go to them for help. The form can also be used for those asking for help.. like a wanted ad.

anyone able to help me out?

needs to have their name, apt #, phone #, skills either possessed and/or wanted, questions/comments section.. and anything else that might be useful.
