The need to increase the length...

Malaysian Man Gets Nut Stuck Around His Penis
Nut, penis, you might think I was making this up. But no, I'm not.
A Malaysian welder was attempting to, yes, lengthen his penis prior to his engagement (not even wedding!) the next week by attaching a metal nut to it. The hope was the weight of the nut would pull down and lengthen his organ.
However, the nut became stuck on his penis after an erection. My assumption is the blood couldn't drain the penis, leading to an unending erection. Staff from the Sultanah Aminah hospital had to drain some blood from the penis and cut away a top layer of skin before the nut, according to a Malaysian paper.
Reminds of a season one episode of Rescue Me, when they had a penis measuring contest, and one of the firefighters got a device intended to keep his penis erect stuck on it.
In Rescue Me, the device had to be cut off.
"The patient is now recovering and we hope to discharge him today (Sunday)," hospital director Daud Abdul Rahim told the paper.
