Usually
I still haven't gotten use to the Tucson/Marana Cop cars yet they have different cars/headlights so I can't quite tell if they're following me

One of my old roomates and I used to be able to do it by the headlights in the rear view mirrors at night. We had an accuracy of about 85% on average. Now that all the front ends pretty much look the same it isn't as easy, but I still do pretty decent at it.
