Although I never played it, I know plenty of people who play it fairly often.
To tell you how much harder ITG is than DDR in terms of difficulty, one of my friends was once a strictly ITG player but started playing DDR Extreme around summer 2005. Within two weeks, he was able to achieve the score of AA on Max 300 (one of the harder songs in DDR; roughly 550 steps within 100 seconds) and did not even break a sweat.
Apparently, ITG has far, far more stamina draining routines do to much more leg movement. A lot of the step patterns in ITG would make a DDR person cringe at the thought of it...
