Do not perform any work beyond the scope described without contacting the Petco Maintenance Department for further approval. Any work that exceeds the “not to exceed amount” will not be paid without prior approval from the Petco Maintenance Department.
Upon completion of work, this Petco Work Order must be signed by the store manager and stamped with the store stamp. The signed and stamped copy must be mailed along with the invoice to ensure proper payment for work performed. Failure to submit invoice and paperwork within 90 days of work order date will result in non-payment.
