Default Timing question

What does it mean when my 91 acura will run fairly well only with the timing advanced a full inch beyound the 16 timing mark on the crank. Is there a larger problem here or do I need to do somthing to corrrect. The car still studders in lower rpm's from time to time.

You know I thought of the skipping a tooth on the timing belt thing, but I did not want to sound stupid. I did not think that would be possible. Whats the easyest way to check that?
