Default Boiling coolant????

I have a 1992 Integra and until this past weekend noticed the fan had been running long after I shut the car off, sometimes 20 min after. Then this weekend on a trip to the mountains, I stopped the car to hear not the fan running but my coolant boiling. The temp gauge in the car read normal, mid range but the coolant reservoir which was filled to the max level was boiling so much a good bit of the water/ anti freeze was leaking out the top. The way down the mountain there was no boiling or weird temp readings inside.
Any idea what might be wrong, and how much I'd have to shell out to fix it!?
Thanks for any suggestions.

thermostat could be blocking the system or there may be a blockage in the cooling system. i am leaning more towards poor circulation in the system. i could be off though...

The radiator cap is related to pressure, not the boiling point or temp. The thermostat is temp related.
