Default replacing exhaust manifold

I have a 95 GS-R with the infamous exhaust rattle caused by the exhaust manifold. I wanted to know if this design problem had been fixed in the later GS-R's? Perhaps by the 2000 model year or so. I ask because I'm thinking about buying a used exhaust manifold to replace my current one.
In addition, does anyone know exactly which part of the manifold the noise originates from? I hear there is an upper and lower part of the exhaust manifold and want to know which part exactly I need to replace.

are you sure it isn't just a heat shield rattling?

It is the A-pipe, or the pipe leading into the cat. You could replace just the OEM downpipe, or buy a used header from someone for cheap, or just buy an aftermarket header which will rid you of the problem altogether.
