Whats wrong with my car?

Well here is the deal. I have a stock Integra Type R with 70,000 miles on it. When i bought the car it had 47,000 miles on it and it had a ACT stage 2 clutch. A couple of months later it went out at about 56,000 miles. What happend is that one of the springs in the disc got mangled some how. The throw out bearing looked good but we replaced the whole clutch with the same clutch it had in it. Well now it is making almost the same sound again. When the clutch is in its quiet but when u let the clutch out u can hear something spinning loosely and when u push on the clutch again when it is pressed down u can hear the spinning slowing down to a stop. I have always been told its the throwout bearing but last time it was the spring. So now im confused. First ? is how long should ACT clutch last? I drive it daily and put a lot of miles on it. I am in Vtec at least once a day but i dont drive it that hard. Second ? is what could be wrong with my car this time? I had a friend who drove it tonight who also has a R and he said it sounded like a bolt was loose in the flywheel. Of course i am not sure if that would leave my car with the symtoms i am having. Sorry it was so long but i wanted u to have all the info.
