Default 92 Accord torque converter no lock up

I have a '92 Accord and the torque converter is not locking up resulting in higher RPM's during highway driving. I had noticed that over the last year it would occasionally not lock up while driving at highway speeds for several minutes then it would eventually lock up. Now it will not lock up at all. I would like to know if anyone has any idea what the problem would be and how much it would typically cost to repair. Additionally, is there any significant problems that I am causing by driving with this condition besides lower gas milage?
