Default proper prodedure for clearing DTC's from ECU memory

What is the right way to erase the fault memory on the ECU? I know I'm supposed to pull a fuse or fuses, but which one(s)? I've heard that I can pull either the clock/radio fuse, OR the ECU fuse. Will both of them erase the fault memory?

Do I pull the fuse with the ignition turned on? or off? Will it hurt the ECU if the fuse is pulled while the ignition is on? Hopefully someone knows this. Thanks
