Default Slipping gears...what to do?

Greetings
I got a problem with my 96 civic. I have recently noticed that when i shift into 2nd gear sometimes, it grinds. Also, when Im driving in 3rd gear and it sometimes slips out. Does anyone know the cause for this? Since I bought the car, I've never changed the gear oil..could that be the problem? Or is it time to change the cluth plate?
Any help is great!

syncros are bad, had the same problem, 2nd, 3rd, and 4th would grind and 3rd would pop out once in awhile. better to find another tranny to replace for cheap, it'll cost upwards of $1000 to get the tranny fixed itself

but roughly how much would it cost to replace the transmission? ok Im a bit confused... i thought i would just have to replace the cluth plate. But it seems a bit more complicated than that!
