O2 sensor could be Facked up!

 

The IACV is it's own system. It has nothing to to with your problem. The only time it is working is at Idle, unless you code comes on at idle. I would go ahead and check you o2 sensors. I'd focus on the front one, not the one in the cat. I would check all components in the fuel system. go ahead and check the fuel pressure first and see if its within specs. then go from there.
