Just sounds like a bad regulator its not responding to the vac signal. No real fix but get a new one. But if the adjuster nut will changing your psi just set it high and kiss you nice idle goodbye.

The reading are the same on the fuel rail and after the fuel filter. The fuel pressure regulator is after both.

Where are you plugging the vac line into the throttle body or the intake manifold.
