Could be the tensioner wasn't torqued correctly, as state above, worn TB or what happend to me...
A little rock flew in there and chew it up a bit to where it was 3/4 ripped and jumped a teeth.

On that note.. guys put back your upper timing cover, no matter how sexy those useless cam gears look.
