Don't point fingers, don't push her buttons; just express yourself, the problems, what you don't like, and what you see as being the only solution at this time - breaking it off. Women always complain that men don't express their feelings well. Put that in her face and tell her exactly what you've said here, edited to be a little less brash, of course.

If she can't take it then you know what you have to do. She might respond saying that she's had it too and then you have a mutual agreement. And, she might go nuts and try to hack your ear off. Then you also know what you have to do - get a bigger knife. h: Consider even writing it down and reading it when you talk to her. I know my head starts to swim when I get in situations like that and it helps to have it all drawn out created at a time when your mind was solid and you weren't under the pressure of the situation.
