splash guard has nothing to do with it cuz the blower motor is in the passenger cabin. sometimes, debris gets in the motor causing noises but it sound like yours is on its way out if it only turns on on the highest setting. get the electrical checked out before you replace the whole blower cuz you may have to remove the whole dash to replace it.
