You tires somehow got off balance. This in turn caused cupping (uneven tire wear) to tire which in turn contributed to uneven braking which eventually lead to warped rotors (or uneve pad deposits). In a downward spiral, the shaking continued to get worse, thereby worsening all of these things.

You fixed one broken part, the rotors. Now you need to address all the other stuff.

A more rare possibility is the worn/blown bushing as mentioned above.

Like mentioned above also: check even air pressure, check bent rim.

Make sure a seiko watch isn't lost inside the tire.
