Back in the day, I had the same problem with the tape deck of my 96 Accord and the tape adapter for my Sony Discman. I found that if I set up the discman to start playing the first track on the cd and then insert the cassette adapter into the tape deck it would work, but if I tried to stick the cassette adapter with out first turning on the discman it would spit the tape back out. Your cassette adapter might be the same way.
