I bought a B&M shift knob which came with urethane inserts. When you installed the knob, the urethane was cut by the shifter's threads for a perfect fit. And because it's urethane, it maintained the fit.

On the other hand, I bought an Ebay type R replica before and the threads sucked.

B&M is a good way to go.
