I've had to use a jack or crowbar or something to press the taper shaft tightly into the tapered hole so it doesn't spin. That's not too unusual.
Hmmm...I know you know more about this stuff than me...I guess I've just wondered how you can tighten or loosen the castle nut to torque if it rotates...but it does make sense that it would turn. I just know that I replaced the only one that ever turned on me...this thing was turning so easily I couldn't even hand thread the nut on. Replaced it, and the new one held dead still so I could torque to spec.
