Could be a number of things... You may not have a good seal in the headlamp housing. You could have a crack in it which is allowing moisture to get in and fog it up, etc. At some point, water my begin to condensate as well. I don't know what your set up is but make sure you have a good seal and no cracks. If you have a crack you may just have to live with it or replace it. Good luck.
