I have sort of the same problem with my car... but its automatic. The issue with my car is the my alternator is being strained which is straining the motor, but this is only at red lights and only with everything on, they told me because I have several stuff hooked up that I would need a bigger alternator.

But my friend has a stick and they told him that it could be a vacuum problem.
