Yes, a GS-R against an B18A/B car with 404's would be a fair race.
There is no "second" stage of VTEC on a B18C. Either it's "on" or "off."
Yes, the B18A/B will idle worse and have poor emissions results, but it will keep up with the B18C for less money. The cam'ed B18A/B is basically like a B18C with VTEC always activated.
