The special tool for DOHC motors just makes it easier. Just imagine a really deep socket, with a handle attached so you don't need to use a ratchet. Then stick a screwdriver down the hole where the ratchet would have been.

The reason is that for DOHC VTEC heads, the adjustment is down between a bunch of stuff & there isn't enough room for a regular box wrench.
