all you need for a rear disk brake conversion.


are the trailing arms
the hub that's connected to the trailing arm and it holds the disks

you'll need the calipers also.

um... brakes lines (i'd buy the aftermarket steel one)
and the emergency brake cables.

also since you have that **** out.
i would also buy the suspension kit where you replace the bushings with the urethane ones, the rubber crap sucks. i'd def replace all that rubber crap. it's pretty easy to get it done through a machine shop and it shouldn't be that expensive either.
