How much smoke is coming out? And is it pretty constant or just a bit when you first start up in the morning?

Either way I'd say to check your coolant both for its level and any odd coloration. Possibly a bad head gasket which is very likely possibility on a car with those miles.

it sounds like blown head gasket, take a look at your oil, does it appear to be milky in color? The white smoke says head gasket, but the fact you say you arent losing coolant is interesting.

with 277,000 miles something is bound to take a dump. At this point i would question whether or not it is worth it to worry about it or just run it to death.
