Thats a relative question. Personally, Im not a huge fan of the 2005 and up. I dont like the box look and the wheel wells are hideous IMO. Im a huge fan of the 2001-2004 and its the platform for my new full on prerunner project!

Get the 5 speed. I also prefer the pre-2005. The new ones are just too damn big, IMO. At that point you might as well just get a full size (turbo F-150 actually looks pretty promising). As for year, it really doesn't matter, there were really only minor tweaks throughout the 96-2004 generation, and as far as I'm aware the drivetrain of the late model trucks are pretty stout as well.

What is the purpose of the truck? If it's just for hauling crap around, I would get a 4-banger 2wd. Better on gas and surprisingly "fun" to drive :o
