You have to take off the door panel. Here are some instructions...I've done it myself 2-3 times:

1) Remove the sail panel ahead of the side window by snapping it off via force.
2) Remove all screws/snaps. There are 2 screws and one snap. The snap is on the forward side of the door and must be pried off with a small screwdriver. One screw is in the cupholder and one is hidden behind a little circular door on the door handle.
3) Stick a screwdriver under the door panel and, working around from bottom to top, pry up so that the door panel pops off. It's all held on by snaps behind the door panel.
4) Manipulate the door handle through the hole in the door panel
5) You're done

Here's a picture overview: http://www.morningz.com/mz/integra/a..._doorpanel.asp

Sorry for bringing this thread back. I'm trying to remove the door panel to get some new speakers in....

this step

"4) Manipulate the door handle through the hole in the door panel"

I can't get that to work. it feels like i'm going to break my door handle. Isn't there supposed to be a way to do this w/o breaking stuff?

Never mind. you push the handle to the right and it dislodges 2 L shaped plastic tabs and loosens the handle :-P

For those reading this at a later date: Here's a good video for further reference on the entire process:
